Vital Signs Survey

If you live, work or go to school in the City of Grande Prairie, the County of Grande Prairie, the MD of Greenview and area, we invite you to share your views and priorities about your community.

The voluntary survey is open to anyone age 15 and over. (If you are in Grade 9 and not yet 15, you can participate.)

Thank you for adding your voice to the Community Foundation of Northwestern Alberta’s Vital Signs survey. This year, the survey includes some questions about how COVID-19 has affected your well-being. By participating you’ll have a chance to impact your quality of life.
